  4. This column indicates whether the BaBar results described in the document have been approved by the BaBar Collaboration for public presentation.
  5. This column indicates whether the text of the document has been formally reviewed by the BaBar Collaboration.
  6. If this column is checked, then the author represents the BaBar Collaboration alone. Reference may be made to results from other collaborations, but the author has no formal responsibility to present results on their behalf.
